What We Changed to Reduce a Common User Error

During Beta testing, we monitor behavior data across different parts of the product. One particular flow had an error rate that was too high to ignore.

How we found the problem
It was not a user complaint that flagged this. The data did. Our internal monitoring showed that a specific operation path had a consistently elevated error submission rate, meaning a significant portion of users were either completing an action they did not intend to, or immediately reversing what they had just done. Both behaviors together are a clear signal. Something about the design was not working.

What we changed
We approached it from three angles. The first was the flow itself. The original path was too short. Users could complete a submission without having fully processed what they were doing. We restructured the flow to create a natural pause at the critical point, giving users a moment to confirm before they committed. The second was the confirmation step. We added a clear confirmation moment just before final submission. Not a popup, not an extra click for the sake of it, but a deliberate point where users could see exactly what they were about to do. The third was the copy. The original prompt text was vague. Users were not sure what the outcome of their action would be. We rewrote it with one goal in mind: by the time a user submits, they should already know what is going to happen next.

What changed after
The error rate dropped noticeably. More tellingly, the reversal behavior on that path also decreased, which suggests users are going into the submission with more clarity about what they are doing. This was not a large change. But it illustrated something worth saying out loud. Most user errors are not the user’s fault. They are a signal that the design did not give the user enough information or space to make the right call.
